• Why you are requesting immediate cancellation
• Your circumstances
• The date you would like the cancellation to take effect
• Whether you are currently practising, teaching or using your CMA credentials
• Where you currently display or use CMA branding
• A written explanation of your request

You will also be asked to confirm that you understand the changes you must make if your Membership ends.
